                         1.  FORM NO. 4
                         2.  FORM NO. 8
                         3.  Learner License duly passed by the MVI/DSP traffic
                         4.  Driving School Certificate (in case of commercial vehicle license )
                         5.  Original previous License in possession of applicant.
