                        1.  Form of International Driving Permit.

                        2.  FORM NO. 1-A (Medical Certificate from MBBS (Doctor) with self photo attested by the doctor with

                             Blood Group)

                        3.  FORM NO. 4A (Application Form).

                        4.  Fees of Rs.500/- deposited through challan (Form No. T.R.6) in the Treasury Branch of S.B.I., Sector 17,

                             Chandigarh.

                        5.  Three passport size latest photographs.

Attested copies of :

                        1.  Valid Indian Passport

                        2.  Valid Visa

                        3.  Valid Travel Ticket

                        4.  Valid Indian Driving License

                        5.  Residence Proof
